Mumbai (Maharashtra) [India], August 4: A treatment that was, until recently, available only through international hospitals is now being offered inside the country and it is changing what “running out of options” means for families dealing with relapsed leukaemia.

There is a particular kind of exhaustion that comes with searching for answers after a leukaemia diagnosis. You read everything. You ask everyone. India now has its own approved product for this therapy. India now has an approved indigenous CD19 CAR T-cell therapy , making this advanced treatment more accessible for eligible patients within the country.

The process spans several weeks. Cells are collected in a short-stay procedure that most patients find far less daunting than they expected. Those cells are then engineered over two to four weeks in a lab. When they are ready, the patient goes through a short course of conditioning treatment before the infusion itself, which takes only a few hours. After that, the medical team monitors the patient closely for about two weeks as the immune response builds.

What happens after discharge matters as much as what happens during treatment. Check-ins, blood tests, and bone marrow assessments typically continue for years, this is not a therapy where a family is handed a discharge summary and left to manage the rest on their own.

Centres delivering this therapy are upfront with families about what to expect. The main side effects are well documented and are watched for closely from the moment of infusion. Most patients manage them without serious complications when the treating team has handled these responses before, which is precisely why the therapy is currently offered at only a small number of specialised centres rather than every hospital.

Because CAR-T therapy is still available at only a handful of hospitals in India, the details of where it is delivered matter almost as much as the therapy itself. A few things consistently come up when families and doctors discuss what makes a difference:

  • Continuity of care. The specialists who assess a patient are ideally the same ones who treat them through every stage, rather than passing the case between departments.

  • Integrated facilities. CAR-T therapy, bone marrow transplantation and other blood cancer treatments are best delivered under one roof, so families are not moving between hospitals for different parts of the same treatment plan.

  • Multidisciplinary approach. CAR-T therapy involves more than a single specialist. A coordinated team of   doctors, nurses, and allied healthcare professionals works together to support patients from evaluation and treatment through recovery and long-term follow-up.

  • Purpose-built infrastructure. Sterile environments and specialised isolation rooms are not something a general hospital ward can simply add on, they need to be designed for this kind of work from the outset.

  • Long-term follow-up. Because outcomes are monitored for years after treatment, centres with a structured follow-up programme tend to catch complications earlier.
